Ferex Inj. is an intravenous iron-replacement product (iron sucrose) used to correct iron deficiency when oral iron isn’t an option. It delivers iron directly into the bloodstream, making it suitable for patients who can’t tolerate or don’t respond well to oral iron. It presents as a brown solution in a clear, colorless ampoule.

Iron supplementation in iron-deficient patients who: 1. Cannot take oral iron, or whose oral treatment is unsatisfactory; or 2. Have poor compliance with oral medication, making treatment effect doubtful. Use only when iron deficiency is confirmed by appropriate tests (serum ferritin, hemoglobin, hematocrit, RBC count, or RBC indices — MCV, MCH, MCHC).

Протокол

Professional administration: • Total dose = total iron deficit, calculated individually: body weight (kg) × (target Hb − patient Hb)(g/L) × 0.24 + storage iron. (<35 kg: target Hb 130 g/L, storage iron 15 mg/kg; ≥35 kg: target Hb 150 g/L, storage iron 500 mg.) • Adults: 5–10 mL (100–200 mg iron) per dose, 2–3×/week, per Hb level. • Children ≥3 yrs: max 0.15 mL/kg (3 mg iron/kg), 2–3×/week. Not recommended under 3 years. • IV drip (preferred): dilute only in normal saline (5 mL → max 100 mL; 10 mL → max 200 mL). Infusion time by dose: 100 mg ≥15 min, 200 mg ≥30 min, 300 mg ≥1h30, 400 mg ≥2h30, 500 mg ≥3h30. Max single dose 500 mg iron; max 7 mg/kg/week. • Slow IV injection: ≤1 mL/min, max 10 mL (200 mg) per dose, over ≥10 min. • Cannot be given by intramuscular injection. Single use only — discard any remainder. Must be administered where CPR/anaphylaxis care is available; observe patient ≥30 min after. Do NOT use in: hypersensitivity to iron-carbohydrate complexes; anemia not caused by iron deficiency (e.g. hemolytic, B12-deficiency/megaloblastic, thalassemia); iron overload (hemochromatosis, hemosiderosis); bacteremia; first-trimester pregnancy; chronic alcoholism. Use with caution in: heart disease, autoimmune/inflammatory disease (SLE, RA), liver impairment, asthma/eczema/atopy, acute or chronic infection, and the elderly. Warnings: rare but potentially fatal hypersensitivity/anaphylaxis (collapse, low blood pressure, breathing difficulty); common effects include headache, dizziness, nausea, muscle/joint pain, injection-site reactions, and temporary discolored urine. Do not mix with other drugs or with oral iron (wait ≥5 days after the last injection before starting oral iron).
